  • SICOT initiatives to improve the quality of orthopaedic care in developing countries

The quality of healthcare services varies widely among the countries of the world. The widest gap of care, however, is that between the developed and the developing countries. In the 21st century, the quality of and access to health care is not just a regional or national problem, it is an international one. Rich and poor, urban and rural populations in every country have a right of access to uniformly high-quality health care. The national health care system of most countries is based on two pillars: the physical infrastructure of health care delivery systems, and the health care providers. Although both are equally important, the health care providers have the greater effect in shaping the future of health care services. Read more...

Potency of double-layered Poly L-lactic acid scaffold in tissue engineering of tendon tissue
Atsuyuki Inui, Takeshi Kokubu, Takeshi Makino, Issei Nagura, Narikazu Toyokawa, Ryosuke Sakata, Masaru Kotera, Takashi Nishino, Hiroyuki Fujioka & Masahiro Kurosaka

A successful scaffold for use in tendon tissue engineering requires a high affinity for living organisms and the ability to maintain its mechanical strength until maturation of the regenerated tissue. We compared two types of poly(L-lactic acid) (PLLA) scaffolds for use in tendon regeneration, a plain-woven PLLA fabric (fabric P) with a smooth surface only and a double layered PLLA fabric (fabric D) with a smooth surface on one side and a rough (pile-finished) surface on the other side. • Case of the Month

A 35-year-old young man was involved in a motorcycle accident 2 weeks ago, where he suffered a massive heamato-pneumothorax.

After improvement of his chest injury 2 weeks after trauma, he is presented to the orthopaedic department for management of his sacral injury (Fig. 1). He reports paraesthesia of both feet.

Fig. 1: At time of presentation the a.p. X-ray shows sacral fracture on the left and right side.

In view of this information, what might be a good diagnostic possibility at this time?

    • X-ray outlet view
    • pelvic CT scan
    • 3-dimensional reconstruction CT scan
    • MRI

  • Pain Management in Children: Ibuprofen equal to Acetaminophen & Codeine
    by Dr Ali Masoud

Pain control is a crucial part of fracture management, yet, many children receive inadequate analgesia in the acute patient care setting. This was validated by a study of 1,210 children which has shown that few to none of them received analgesia for even routine procedures, such as venipuncture or nasogastric tube placement. However, as the practice of pain management for children in the acute setting is changing, pain should be assessed as the "fifth vital sign" during triage, and validated scales can be used to assess a child's degree of pain. A study by Friday JH et al compared 2 common analgesics that are often given to children with acute injuries of the extremities, and demonstrated that ibuprofen was similar to acetaminophen with codeine on all major study outcomes. Acad Emerg Med. 2009;16:711-716.

  • Vertebroplasty is a safe and effective treatment for vertebral compression fractures, a multicenter study
    by Dr Ali Masoud

"In our large series of patients, vertebroplasty proved to be a safe and effective treatment, resulting in same day, dramatic improvement in pain," said Stefano Marcia, M.D., interventional radiologist at San Giovanni di Dio Hospital in Cagliari, Italy. Dr Marcia and his colleagues retrospectively evaluated the clinical outcomes of vertebroplasty in 4,547 patients with a total of 13,437 fractures treated in six different EVEREST centres.

The patients were affected by severe painful osteoporosis, painful vertebral tumours (metastases or myeloma) with high risk of fracture and symptomatic vertebral angioma. Inclusion criteria were VCF with bone edema on MRI, local back pain with single-point pressure on examination, and poor or no response to medical therapy.

Through a one-year follow-up, pain relief was evaluated using an 11-point visual analog scale (VAS) physical examination and X-ray at 48 hours and one month, and physical examination at three months and one year.

Within 48 hours, (88.06 percent) experienced clinically and statistically significant pain relief. The pain remained improved at each follow-up evaluation up to 12 months. No major neurologic complications were encountered. Venous leakage was the most frequent mild one (20.5 percent). This study is an important clue for validating the vertebroplasty as a safe and effective treatment with a low complication rate. Read more...

JUST BECAUSE AN INCISION STARTS OUT SIMPLE
DOESN'T MEAN IT ENDS UP THAT WAY.

Complications such as surgical site infection, dehiscence, or even hematoma and seroma may progress into problems for high-risk patients. The PREVENA™ INCISION MANAGEMENT SYSTEM is the first and only negative pressure product designed specifically for management of incisions at risk of post-operative complications. The Prevena™ System helps draw incision edges together and may reduce the likelihood of surgical site infection and dehiscence, while stimulating perfusion, and acting as a barrier to external infectious sources. For more information, visit www.kci-medical.com.
By KCI, leaders in negative pressure therapy.

NOTE: Specific indications, warnings, precautions and safety information exist for the PREVENA™ Incision Management System. Please consult a physician and product instructions for use prior to application.
